STOP PERIOD BACK PAIN

Hi, I’m Dr. Molly with Your Goals Physical Therapy. Okay women, let’s have a very real conversation. Your monthly period does not have to come with back pain. I have heard from several women that say that their back hurts every month. They thought it was just part of the deal.  The pain changed monthly. Sometimes it would be kind of an ache across their low back, sometimes the pain was at just one point. They just thought this was normal and there was nothing they could do.

One woman, Rose, went into a lot more detail. She told me that she actually went to one doctor and they told her that her hips were misaligned, one leg was longer than the other, but that was just her state of being. She’s just going to have one leg longer than the other, or she’d have to keep coming every couple months to have an adjustment done so that they would get rid of it for her. Her general practitioner did an x-ray of her back and told her, “Well, there’s not really anything going on in your back. So you just kind of have to live with this back pain every month.” And so here she is at the age of 50, and now she is just fed up and she’s been scouring the internet to find out how to stop this back pain, because it’s not about the pain anymore.

She’s made it through pregnancy and several decades of discomfort. She was worried because it was starting to get worse. It’s starting to affect her job, not being able to sit for as long as she needs to.  She’s not able to go to the grocery store. She’s not able to do basic housework. She can’t go to the gym and workout the way she wants to. She can’t go for a walk with her neighbors because during the week of her period, she has sharp pain that hurts every time she puts weight on one leg. She can’t sit for long periods because the pain in her back would get so bad that she’d have to stand up and shift positions.

She’s tried to lift one knee and she’s tried to do all these crazy positions but nothing takes away the pain. At least it goes away after about a week, but every month this happens. So she came in and we started talking and I was able to kind of explain what they were telling her when they told her about that leg length discrepancy.

She’s like, “So you can fix that?” Yes, that is definitely something that can be fixed. What they’re talking about is that every month we, as women, get an influx of hormones and those hormones tell every ligament in our body to relax. This is our preparation for having kids. And our bodies don’t necessarily know that I’m 50. I’m not going to be getting rid of you soon so we don’t need to prepare them for kids every month. It just does it. So our ligaments naturally relax. If one side of our hip is tighter than the other side, or there’s some kind of instability in our pelvis, that’s normally there. Then perhaps we go up a flight of stairs or we take a step off to something like a curb.  Causing us to pull a little harder on one side of our body.

Causing one leg worked a little harder than the other side. That  normal daily task can cause a little shift in your pelvis. If it shifts too much, too much being, up to the discretion of our bodies, a muscle spasm will happen. And that is what you’re getting. You’re getting that searing pain or that ache across your back because your muscles are trying to do their job. They’re just on overtime. They don’t know that they can relax. And so really the trick is how, what is causing all the instability.

Is there global weakness? Is there one muscle that’s weak? Is there something that’s tight? Finding that out and then correcting it because once you know the root cause, like which muscles are actually causing the problem or which ligaments are actually causing you the problem, then you just go and fix those things.
